Adelaide Primary Health Network

Adelaide PHN is a federally funded organisation who works together with the primary health care sector to improve health outcomes for all people living in the Adelaide metropolitan region.  PHNs have been established with the key objective to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and improve coordination of care to ensure patients receive the right care in the right place at the right time. Adelaide PHN covers all the Adelaide metropolitan region, from Sellicks Hill in the south to Angle Vale in the north, and from the beaches in the west to the foothills in the east. This area is home to 1.2 million people, most of the South Australian population.
